Insurance
Dr. Craddock does not participate with any insurance carrier. We will provide you with a statement you can submit to your insurance carrier for reimbursement.

Medicare
We have opted out of the Medicare program. Most labs are covered by Medicare. Patients referred out for radiological and specialty services are covered under Medicare. Most patients can file with their secondary carrier for reimbursement.

Workers’ Compensation
We do not participate with Workers’ Compensation program.

Tricare
We do not participate in the Tricare program. Patients pay at the time of service for their office visits.
